Medical Records

Looking for medical records or birth registry information?

We offer two ways to access your medical record:


MyHealthOne Portal:

MyHealthOne consolidates many common tasks into one secure, easy-to-use online patient portal. It gives you access to most of your medical records on your desktop computer, laptop, tablet or smartphone 24 hours a day.

*Note that some medical records may only be available through our hospital Medical Records office.

Paper Copy of Medical Records:

Please complete the following steps to obtain a paper copy of your medical records:

  1. Print and complete the Medical Records Release Form.
  2. Complete, sign and date the form. In order to verify your identification and validate your authorization, we require that you include a legible copy of a valid photo I.D. (e.g., driver’s license, military I.D. or state I.D.).
  3. Mail, fax or personally deliver your paperwork to the facility where you were treated.

Please note: Medical Records copied for reasons other than continuity of care are subject to a copy fee (O.C.G.A. 31-33-3).

Please allow 5-10 days for processing from date of receipt of the completed authorization. Federal law permits up to 30 days.

Customer Service Hours

Please bring the completed authorization form and your photo ID to the HIM/Medical Records Department. The ROI accepts calls Monday through Friday from 8:00am to 5:00pm.

Birth Registry

Birth Certificates are issued by the Georgia Department of Human Services (Georgia Vital Records). You can find out information on how to request a birth certificate by visiting dph.georgia.gov/birth-records or by calling Georgia Vital Records at (404) 679-4702. If you have specific questions regarding a birth registry that is in process, please contact the Facility Birth Clerk at (770) 736-2411.


Urgent Requests, Records for your Physician

For immediate continuity of care, your healthcare provider can request records.

The physician office must fax a written request on their letterhead to (855) 616-3822 indicating the patient's name, date of birth and date of visit in the facility and place "STAT" at the top of their request. For assistance call (877) 403-8825.


Insurance, Attorney, Disability and Third Party Requests

Requests should be sent from your insurance company, attorney, or Disability Determination Service and mailed to the address below. We cannot accept faxes and email requests.
